3 a.m. on a Monday night, your words
rive my skin like
pavement when an earthquake
strikes

and I swear these fault lines will never
piece perfectly back
together

leaving me in fragments, body parts
like continents

whose grounds will never again be
walked by you

because you’ve found new
territory,
a frontier in the stars

to which my dusty fingers may never
reach

i love the relation to an earthquake, and especially the parts, "3 am on a Monday night" and "and I swear these". It adds concrete concepts that are relateable and a sort of teen angst. your choice of line separation in the first and third stanzas really contribute to the fragmented feel of the poem. Beautiful!
